Mommy & Me Music Class

Waiver & Release of Liability

This document outlines the terms and conditions for participation in Rockshop Music’s Mommy & Me Music Class (hereafter referred to as “the Class”). Please read carefully. Your acknowledgment and agreement are required for participation.

1. Assumption of Risk

I understand that participation in the Class involves physical activity, singing, movement, and the use of musical instruments. I acknowledge that such participation carries inherent risks, including, but not limited to, personal injury, illness, allergic reactions, and property damage. I voluntarily assume full responsibility for these risks for both myself and my child.

2. Medical Authorization

In the event of an emergency, I authorize Class staff to seek necessary medical treatment for my child and/or myself and to administer basic first aid if needed. I understand that every effort will be made to contact me or my designated emergency contact before taking such action. I agree to be financially responsible for any medical treatment or emergency services deemed necessary.

3. Release of Liability

I, the undersigned, hereby release, waive, and discharge Rockshop Music, its staff, directors, volunteers, and facility providers from any and all liability, claims, or demands for personal injury, illness, or property damage sustained during Class activities, whether caused by negligence or otherwise.

4. Photo/Media Release

I understand that photographs, video, or audio recordings of myself and/or my child may be taken during the Class. I grant permission for these to be used for promotional purposes by Rockshop Music, including social media, flyers, and websites, without compensation or further approval. (You will select your preference for this on the registration form.)

5. Behavior & Participation Policy

I understand that children and parents are expected to behave respectfully, follow all Class rules, and adhere to the instructions of Class staff. Rockshop Music reserves the right to dismiss any participant without refund if behavior poses a safety risk or creates a significant disruption to the program.

6. Communicable Illnesses

I understand that despite precautions, the Class cannot guarantee protection from communicable illnesses, including COVID-19. I agree not to attend with my child if either of us is experiencing symptoms of a contagious illness or has tested positive for one.
