Music Camp Waiver & Release of Liability

This document outlines the terms and conditions for participation in [Your Rockshop Name] Music Camp (hereafter referred to as “the Camp”). Please read it carefully. Your acknowledgment and agreement to these terms are required for participation.

1. Assumption of Risk

I understand that participation in the Camp involves physical activities, group interactions, and the use of musical instruments and equipment. I acknowledge that such participation carries inherent risks, including, but not limited to, personal injury, illness, allergic reactions, and property damage. I voluntarily assume full responsibility for these risks on behalf of my child.

2. Medical Authorization

In the event of an emergency, I authorize the Camp staff to seek necessary medical treatment for my child and to administer basic first aid if needed. I understand that every effort will be made to contact me or my designated emergency contact before taking such action. I agree to be financially responsible for any medical treatment or emergency services deemed necessary.

3. Release of Liability

I, the undersigned, hereby release, waive, and discharge [Your Rockshop Name] Music Camp, its staff, directors, volunteers, and facility providers from any and all liability, claims, or demands for personal injury, illness, or property damage sustained by my child while participating in Camp activities, whether caused by negligence or otherwise.

4. Photo/Media Release

I understand that photographs, video, or audio recordings of my child may be taken during the Camp. I grant permission for these to be used for promotional purposes by the Camp, including social media, flyers, and websites, without compensation or further approval. (You will select your preference for this on the main registration form.)

5. Behavior & Dismissal Policy

I understand that campers are expected to behave respectfully, follow all Camp rules, and adhere to the instructions of Camp staff. The Camp reserves the right to dismiss any camper without refund if their behavior poses a safety risk or creates a significant disruption to the program.

6. COVID-19 and Communicable Illnesses

I understand that despite precautions, the Camp cannot guarantee protection from communicable illnesses, including COVID-19. I agree not to send my child to Camp if they are experiencing symptoms of a contagious illness or have tested positive for one.
